Firstly, please Download Test Submission Form here and complete it.
Step 1 – Priority Results Service
Download Priority Test Submission Form, print it out, and complete it.
Step 2
Please take a minimum of five strands of hair with a follicle (from the root) and return with a completed Test Submission Form to our laboratory as soon as possible.
** COUPLES & FAMILY TESTS **
Please ensure hair samples are individually packaged (per person) into small sealable bags.
Step 3

Results are emailed in PDF format within five working days from receipt of the hair sample and test submission form.
